U bent met uw huisdier door uw dierenarts verwezen voor een gebitsbehandeling. Om een afspraak te maken zullen we u eerst registreren in ons systeem. Dit kost soms wat tijd. Wij willen die afspraak graag met u maken, maar zijn tijdens ons werk niet altijd direct in staat om u aan de telefoon te helpen. Om die reden hebben we onderstaand formulier gemaakt. U kunt dit formulier invullen en verzenden. Wij kunnen dan alvast een patientenkaart voor uw huisdier aanmaken. Wij nemen daarna zo snel mogelijk contact met u op om de afspraak te plannen.

Unfortunately we cannot always help you straight away. To help the registration process we have created a form for you to fill out and send to us. Our staff can create your pet's patientfile with the supplied information and then contact you as soon as possible to schedule the appointment.
